I am not sure if it would work under a thick mattress. I really have not tried many mattresses but need to do something soon.

Hi Linda,
Adjustable bed bases are great for sleep apnea and breathing problems, in general....and also for blood circulation to the legs.
They are a very poor choice for back pain, as sleeping on an angle will likely worsen your back pain.
I suggest getting a couple of bed risers, in lieu of the adjustable bed base.
This will tilt the head of the bed and relieve your sleep apnea without hurting your back.
