Hi Rich,
The latex topper is the first thing to get, because it could easily do the job and cost a lot less than a new mattress.
It your wife still needs a new mattress, then one with a pocketed coil will help to relieve the pressure points.
And yes, you would use the latex topper with the new mattress.
The latex toppers will last form many years.
First, is the latex topper...2" of soft Talalay Latex, from EverEden or PureBliss

The soft Talalay Latex toppers provide excellent surface comfort, and may very well be all you need.
If a mattress is needed, you can use that same latex topper with either, the Adrian or Adara mattresses...shown here:
