Hi William,
Your biggest shopping challenge is in dealing with your sensitivity to change. All mattresses begin to wear out the moment you start using them...much like a machine. Unlike a machine, there is no routine maintanence that will keep it performing in tip top shape. It just wears. The padding compresses and the coils weaken...little by little...day by day.
Most people won't notice the difference between how the mattress felt when it was knew, compared to a year or two later...depending on the quality of the mattress and body weight. You, on the other hand, will notice the more subtle changes. Finding a comfortable mattress is easy. Finding one that will remain comfortable over a number of years is more difficult. For you, it'll be damn near impossible. Given that your back is sooo screwed up, (no pun intended), I'd have you look for a solid, basic, firm mattress. It isn't likely to cost much and you shouldn't be thinking of this purchase as an investment. An expensive mattress isn't going to improve with age or increase in value. Consider resolving yourself to buying a new mattress every few years instead of trying to buy one for a long term.
Besides getting a good basic mattress, think about adjusting the surface feel to your liking with a topper.
